When treatment has to happen outside South Sudan

Juba Teaching Hospital sits at the top of South Sudan's referral system and manages a wide range of conditions. Some cancers are treated there, and the private sector in Juba has expanded its diagnostic capability.

What the country does not have is a dedicated cancer institute, a radiotherapy service, or a transplant programme. South Sudan's Ministry of Health has said publicly that patients needing further cancer treatment are referred outside the country.

For those conditions, treatment means travelling. The question is where, and what the complete cost will be.

Arranging treatment from abroad

Many South Sudanese families arrange treatment for relatives from outside the country, and the process is designed to work that way.

Reports can be sent from anywhere. A specialist reviews them and returns a written opinion and an itemised cost estimate in US dollars, covering treatment, accommodation and flights for the patient and one companion.

The hospital invitation letter needed for the visa is issued at the same time, and provided free.

That means the person paying receives one complete figure to plan around, rather than a series of requests as the treatment progresses. Ask for that from any facilitator, including this one.

What India offers

Volume, first. India treats over 700,000 international patients a year, and its cancer, cardiac and transplant units operate at case numbers that produce real sub-specialist experience.

Cost, second — as a complete figure. A cardiac bypass at an accredited Indian hospital is quoted at around US$5,500, and accommodation near major Indian hospitals is inexpensive by regional standards.

Language, third. English is the working language of Indian hospital medicine, so consultations, consent and discharge instructions all happen without an interpreter.

And familiarity. Indian medical teams have run clinical camps in Juba in partnership with local hospitals, and Indian medicine is not an unknown quantity there.

Healthcare quality and accreditation

Accreditation is the one quality signal you can check independently before travelling anywhere.

Joint Commission International accreditation involves external audit of clinical protocols, infection control, medication safety, surgical checklists and patient rights. Fewer than 1% of hospitals worldwide hold it.

Apollo, Fortis Memorial Research Institute, Medanta and Kokilaben Dhirubhai Ambani Hospital are among the Indian hospitals that do, alongside India's national NABH standard, held by 839+ hospitals.

India and South Sudan

India has maintained a resident Embassy in Juba since 2012, upgraded from a consulate opened in 2007, so visa matters are handled locally rather than from a neighbouring capital.

India has contributed the largest national contingent to the United Nations mission in South Sudan, including medical personnel, over more than a decade.

Training links are active. South Sudan uses its ITEC training allocation heavily, and there is an alumni network of South Sudanese professionals who have studied in India.

In March 2024, a team of doctors from Indian hospitals ran a medical camp in Juba in collaboration with a local hospital.

The medical visa and the journey

Visa applications are made online through the Government of India portal, with the Embassy of India in Juba processing applications locally.

Two documents carry the application: a hospital invitation letter and an itemised treatment cost estimate. We provide both free once a specialist has reviewed the reports.

There are no direct flights. Patients travel from Juba with one or two connections, commonly via Addis Ababa, Nairobi, Cairo or a Gulf hub, reaching India in roughly 14 to 20 hours.

For a frail patient, layover length and total journey time are clinical questions. We plan the routing around the condition rather than the fare.

Transplant in India — the legal position

Indian law permits organs only from living related donors — parent, sibling, child, spouse or grandparent — with documented proof of relationship. Every case is reviewed by a hospital authorisation committee before surgery proceeds.

Organs cannot be bought or sold in India. Commercial organ trade is a criminal offence and no accredited hospital will source a donor for a patient. Any agent offering to find you a donor is operating illegally and should be reported, not engaged.

The donor travels with the patient and undergoes full medical, surgical and psychological assessment before being cleared. Where a child is a potential donor, additional legal protections apply and paediatric donation is tightly restricted.

What to expect, and coming home

The patient is met at the airport and taken to accommodation near the treating hospital. Pre-admission investigations and the first consultation usually happen within a day or two of arrival.

Radiotherapy runs as daily outpatient treatment over several weeks, so accommodation and living costs for the whole period belong in the estimate from the start.

Before departure the treating team prepares a full discharge package for the doctors in Juba, and we confirm which medicines are available in South Sudan before the patient flies rather than after.

Video consultations with the Indian consultant can be arranged so recovery is monitored without a second journey.

Callout — when not to travel
