The Tissue vs Mechanical Valve Decision for African Patients


This section is specific to African patients having valve replacement surgery. It is a decision that is made differently for African patients than for Western patients, and it is important that the conversation happens before surgery rather than after.

A mechanical valve lasts a lifetime but requires lifelong warfarin — a blood-thinning medication that needs regular INR blood tests to ensure the dose is correct. In countries with reliable, affordable pathology services, this is a manageable lifelong commitment. In parts of sub-Saharan Africa where regular INR testing is difficult, expensive, or unreliable, a mechanical valve creates a serious long-term clinical problem.

An undertreated mechanical valve patient — one whose warfarin dose is too low because their INR is not being monitored — is at risk of valve thrombosis, which can be fatal. A patient on too much warfarin without monitoring is at risk of serious bleeding.

These are not rare complications. They are the predictable consequence of a mechanical valve in a patient who does not have reliable access to INR monitoring.

The tissue valve avoids this problem entirely. It requires no long-term anticoagulation after the first few months.

It lasts 10–20 years, and for patients over 60, the durability is usually sufficient for their remaining lifespan.
