What Actually Moves Your Final Cost
The final cost of radiation therapy is driven by five things: the technique, the number of sessions (fractions), the cancer type and stage, whether radiation is combined with chemotherapy or surgery, and the hospital chosen.
The technique and the session count together explain most of the difference between two similar-looking quotes.
Two patients having "radiation" for the same cancer can end up with very different bills. These are the levers that move the number.
The technique. A conventional linac course and a CyberKnife course aren't the same treatment. Precision costs more to deliver, so it costs more to book — which is why radiosurgery sits above standard external beam.
The number of sessions. Radiation is delivered in fractions. A short SBRT course might be 3–5 sessions; conventional treatment can run 20–35. More sessions means a higher total, even on the same machine.
The cancer and its stage. A small, contained tumour is simpler to plan and treat than an advanced one needing a larger field, more careful planning, or combined treatment.

Fewer, higher-dose sessions (as in SBRT) can cost more per session but less overall; longer conventional courses spread a lower per-session cost across many visits.

Radiation therapy follows a clear sequence: consultation, a planning CT scan (simulation) with an immobilisation mould, treatment planning and quality checks by a medical physicist, then daily delivery.
Each session lasts about 10–30 minutes, is usually given five days a week as an outpatient, and is painless.
Knowing what actually happens removes most of the fear. Here is the path from first consult to final session.
1. Consultation and decision. The radiation oncologist reviews your scans and reports, confirms the technique, and explains the plan and the number of sessions you are likely to need.
2. Simulation and immobilisation. A planning CT maps the exact target. A custom mould, mask or headrest is made so you lie in precisely the same position at every session — accuracy depends on it.
3. Planning and quality assurance. Physicists and dosimetrists build a plan delivering the maximum dose to the tumour and the minimum to healthy tissue, then run quality checks before anything is delivered.

When Radiation Is Used, and Why
Radiation therapy is used in four main ways: as the primary curative treatment, before surgery to shrink a tumour, after surgery to clear remaining cancer cells, and as palliative care to relieve symptoms in advanced cancer.

Primary (definitive) treatment. For some cancers — cervical, and many head and neck cancers — radiation, often alongside chemotherapy, is the main curative treatment, with no surgery at all.
Before surgery (neoadjuvant). Radiation can shrink a tumour first, making surgery safer and more complete. This is common in rectal cancer.
After surgery (adjuvant). It clears cancer cells that may remain and lowers the chance of the cancer returning — routine after breast-cancer surgery, for example.

Radiation side effects are usually temporary and specific to the area treated — skin changes, fatigue, and, depending on the site, nausea or hair loss in the treated area.
Tell your care team early about anything severe or worsening; most effects are managed with medication and simple aftercare.
