Hospitals rarely price IGRT and VMAT as separate products. In practice you are quoted a plan, and image guidance or arc delivery is part of how that plan is delivered.

That is why quotes overlap so much. What changes the number is the complexity of your case, not the label on the technique.

What IGRT Is and What It Adds

IGRT stands for Image-Guided Radiation Therapy. Before or during each session, the team takes an image to confirm the tumour's exact position and corrects for any drift, so the dose lands where the plan intended.

Here is the problem IGRT solves. Your plan is built from one CT scan taken days earlier. By the time you are on the table for session eighteen, things have moved.

A fuller or emptier bladder shifts the prostate. Breathing moves a lung tumour. Weight change alters the contours the plan was drawn around.

Without imaging, the answer is to treat a wider margin around the tumour to be safe — which means more healthy tissue receives dose.

IGRT lets the team see and correct before switching the beam on. That allows tighter margins, which is the real clinical benefit.

The imaging itself is quick — usually a cone-beam CT or a pair of X-rays taken while you lie in position. It adds a few minutes to each visit.

What VMAT Is and Why Sessions Are Shorter

VMAT stands for Volumetric Modulated Arc Therapy, often sold as RapidArc. The machine delivers radiation continuously while rotating around you, instead of stopping at fixed angles, which cuts each session to roughly ten to fifteen minutes.

Standard IMRT stops at each planned angle, delivers part of the dose, moves to the next, and repeats. VMAT does it in one continuous sweep.

Three things change at once as the machine turns: the beam's shape, its intensity, and the speed of rotation. That is what makes it a single fluid delivery rather than a series of stops.

Shorter sessions are not just about comfort. Less time on the table means less chance of you shifting mid-treatment, which supports accuracy.

For a patient facing thirty visits, the difference between forty minutes and twelve is substantial — especially when you are already tired from treatment.

If a hospital quotes VMAT where another quotes IMRT at a similar price, that is generally good value rather than an upsell.

IMRT, IGRT and VMAT are not three competing machines. IMRT is the planning method, IGRT is position verification added to it, and VMAT is a faster way of delivering it. Many modern plans use all three together.

Which Cancers They Treat

IGRT and VMAT are used for prostate, bladder, lung, gynaecological, head and neck, brain, spine and gastrointestinal cancers. Image guidance is most valuable where the target moves between or during sessions.

Prostate cancer. The strongest case for IGRT. The prostate moves with bladder and bowel filling, so daily verification allows a tighter margin and spares the rectum.

Lung cancer. The tumour moves with every breath. Image guidance, sometimes with breath-hold technique, keeps the dose on target.

Bladder and gynaecological cancers. Organ filling changes shape and position day to day, which is exactly the problem imaging solves.

Head and neck cancers. VMAT suits the complex wrap-around shapes here, and shorter sessions are easier when wearing an immobilisation mask.

Brain and spine tumours. Sub-millimetre accuracy matters when the spinal cord or optic nerves sit alongside the target.

Is the Extra Cost Worth It?

Image guidance is usually worth the extra cost when the target moves — prostate, lung, bladder and gynaecological cancers especially. For a stable target treated with a wide safe margin anyway, the benefit is smaller.

We would rather say this plainly than sell every patient the most expensive option. Not everyone needs image-guided VMAT.

Where it clearly earns its cost: moving targets, tumours pressed against a critical organ, long courses where daily accuracy compounds, and re-treatment of an area that has had radiation before.

Where the case is weaker: short palliative courses aimed at relieving symptoms, and straightforward targets where standard technique already achieves the goal.

The right question is not "which is most advanced" but "what does my tumour do between sessions". Your oncologist answers that from your scans.

What's Inside the Price — and What Isn't

An IGRT or VMAT package usually covers the consultation, planning CT, immobilisation mould, treatment planning, quality checks, the daily imaging and all radiation sessions. It usually excludes diagnostic scans, chemotherapy, side-effect management and accommodation.

One point specific to IGRT: check whether the daily imaging is bundled or charged per session. Across thirty sessions that distinction matters.

Sessions, Timing and How Long You'll Stay

A typical IGRT or VMAT course runs 20 to 35 sessions, five days a week over five to eight weeks. VMAT sessions take about 10 to 15 minutes. International patients should plan a stay of roughly six to nine weeks.

The number of sessions is set by the cancer, not the technique. VMAT makes each visit shorter; it does not usually shorten the course.

Add about a week at the start for consultation, scans and planning, then a few days at the end for a review before flying home.

Weekends are normally rest days. Most patients feel well enough to rest, work remotely or see a little of the city between visits.

Some cancers can be treated with fewer, larger doses on a shortened schedule. Where that is clinically appropriate it cuts both the stay and the total cost.

Budget roughly USD 20–100 a night for accommodation, USD 8–15 a day per person for food, and USD 3–8 a day for local transport. Over a six-to-nine week course these add up to a significant share of the total trip.

Because VMAT sessions are short, many patients find the daily journey takes longer than the treatment itself. Staying close to the hospital is worth paying a little more for.

Approximate and city-dependent. Halal, vegetarian and other dietary requirements are widely catered for near the major hospitals.

Side Effects and Aftercare

Side effects are usually temporary and limited to the area treated — skin changes, fatigue, and site-specific effects such as bladder or bowel irritation. Tighter margins from image guidance generally mean milder effects than older techniques.

If any symptom feels severe, comes on suddenly, or frightens you, contact your care team straight away rather than waiting for the next appointment.

Effects usually build gradually across the weeks rather than appearing at once, and fatigue often peaks toward the end of the course.

For pelvic treatment, you may be asked to arrive with a comfortably full bladder each day. It sounds trivial, but it genuinely improves accuracy — worth doing properly.

Aftercare is practical: keep treated skin clean, dry and out of the sun, stay hydrated, eat well and rest. Attend every follow-up, including after you return home.
